It highlights exactly how much of a shit none of his friends give. I mean, most people, if their friend was having boob jobs, plastic surgery and all that, it would be a big thing. If it didn't happen, you'd want to know why. But Phil has no real friends - they don't give a shit what he does. And vice versa.
Oh, I can absolutely believe that Phil follows through.

The thing is Phils lies are knee jerks to either other trannys lies or IRL events
Tweaker got surgery......so Phil has to get his bottom surgery....or rather he didnt
Tweaker got housing so Phil claimed he was offered housing....but didn't
Phil found out jordman got his balls removed.......so Phil got a boob job.
It's not a situation unique to Phil as his troon circle jerk do same.
So what you get is Phil saying his getting a boob job and every other troon trying to one up him.....but none of them do....and of course Phil is lying but you can't call out Phil because they were also lying.
That tower of lies is the only thing stopping the mass cannibalism of troon society.
