I've never understood making fantasy characters have obvious top surgery scars. If we consider the logistics of those kinds of universes, either the person could magically change their gender (Like changing genitals or transferring your conscious into a different body), or would just have to make do with heavy armor.
If someone in a fantasy era got a mastectomy the same way that one would get in the modern world, the character would likely get septic shock and die from how unsanitary the surgery would be. Not to mention how awful those scars would look.

As sad as I am to know this, the entire Yo-kai Watch community on Twitter is SJWs. A lot of them abandoned the series after it stopped getting localized (thanks to Level-5's incompetent management) and took a few too many turns for the batshit, but some... haven't.
