You're all missing another important thing: He was on meth as well. A dose of heroin with methamphetamine is a speed ball. The methamphetamine counteracts the effects of the opioids in his system, but because of the stimulating effects, you can easily administer a lethal dose to yourself without knowing. The amount of drugs in his system is insane. He probably 'couldn't breathe' because the effects of the meth were wearing off and the massive amounts of opioids in his system were causing him to not be able to breath properly.
