To be fair he didn't have to pay for the bottom surgery except the approximately 145 usd per year for covered health care and a few dollars per day spent in hospital, but FFS is not covered by the health care system and has to be done by a private surgeon.

Edit: to make it clear, Sharkboy is Swedish and I'm talking about Swedish health care

Two spirit was a made up term in the 1990s.

Only the societies with the strictest gender roles had third genders. I also don't understand why they bring this up, because they still labeled them as an other. Which is not what modern TRAs are advocating for. They were never seen as a true women.
