Because they think normal people using their desired pronouns is passing, or even “no one screamed at me in public and called me an ugly tranny” means that the general population believe they are 100% women. They often don’t understand social cues or that women being polite is not the same as thinking they are female.
those are the mega-autist types, the more annoying ones write these loooong paragraphs about how passing is a dated, useless, transphobic concept and they shouldn’t even be expected to make an effort.
Neither of these groups have any chance of being seen as women outside of a Twitter/Reddit hugbox.

It was a reference to post-finasteride syndrome. Everything in medicine is risk vs. reward. Methadone maintenance treatment, despite its side effects, is good because the alternative for many people would be death. Getting on finasteride because you're going bald, well...baldness never killed anyone and tons of men go bald early.
Titty skittles are going to increase your clotting risk and damage your liver at the very least, and the reward seems to be looking like some Silence of the Lambs motherfucker so I doubt it's worth it.

We cant tell someones gender because of their genitals. genitals are not inherently "male" or "female" theyre organs. genitals arent gender identity.
The femimist movement has been working for decades to ensure that women are not defined by their biology. That women are more than 'walking vaginas'. is who you are so much more than your ability to have children or to have a period etc? Great. So why is it so easy to apply this to other cis women whos bodies dont perform this biology? Yet not amab non binary people? Trans Women? Or TGNCI people? Feminism that defines women as nothing more than 'vagina owners' is sending the movement decades backwards. And it also erases the existence of trans men, non binary and intersex people.
Its terf ideology which reinforces the cisheteropatriarchy. Terfs want to fight for THEIR right to not be judged on their bodies, but want to turn around and use that to define OTHERS by their bodies. The truth is that we cant blanket the gender experience of women to a small universal box. And when we do, its a reinforcement of patriarchy. Its erasure. And its gender based violence.
I see terf ideology alive in the lesbian community, where you can hear of "preferences" sounding like "ew i could never have s*x with a penis".. Like we arent already using straps.

As for my “self harm” guess, if this is a dude who wants to be a lady my guess is that he did something to his scrotum. Lots of blood vessels in there that might require an emergency fix, and likely doctors would have done their best at the time to make something that looks like a normal scrotum, so maybe there isn’t enough unscarred skin left to craft labia out of.
