Not sure I understood your last part. Androgenic andropecia is quite rare in even middle-aged and elderly methadone users -- their testosterone is so damn low. I noticed it by observation long before I confirmed it by a literature search.

His point was that the feminizing effects of methadone are worth it because heroin addiction, especially when most heroin is cut with fent, easily leads to death. But you don’t die from baldness, so the negative effects of finasteride aren’t worth it.
