Now - I was searching for the original diagnosis criteria of "Transsexualism" in the 1940s and I cannot find the exact criteria BUT this does article i found setting out the new agenda pretty much allows one to piece it together a little;

That first sentence is the most intriguing. Being transgender
was viewed as a mental health issue, and part of this change of view was to un-attach the stigma. I wish i could find the actual original documents, I've searched around everywhere and can find nothing. Not even a psychologists name or identity - which is kind of suspicious i guess.

Biographies (& other first hand accounts of varying credibility) - I have read books from April Ashley and others, corresponded with a fair bunch of MtFs. The most intriguing find was every single one of these transsexuals had been born with a genetic defect.
Klinefelter's syndrome. It's a genetic condition in which boys are born with extra X chromosome(s) instead of the usual XY - a single 'X' and a 'Y'. More than one extra 'X' and things like mental disabilities are likely to be a more common occurrence*.
Now. I have managed to have these things confirmed from various sources - ONE. A singular lone doctor and his assistants was the only person to ever perform successful male to female sex reassignment surgeries. His name was Dr.
Georges Burou, located in Casablanca. Previously all techniques had failed or slowly killed the patients.
Most transsexual patients suffered from intersex conditions, had been suicidal for reasons of "feeling in the wrong body", Klinefelters being the most common intersex condition.
but Klinefelters can cause low intelligence, mental retardation and even people born with it can have autism too as it is more likely. Do you see my correlation?
Sexual abuse was also a theme that occurs quite a lot, in my outlets of research. One such person was anally raped for being too feminine as a teenager before they transitioned. Trauma is known to manifest with a disconnect between the body and mind functioning, with some symptoms you could describe as being "dysphoria" if the sexual organ was related to the traumatic situation.
A few people said they felt as if they'd been brainwashed. But view it as maybe "internalized" phobias. Gas-lighting (which can be an element of abuse) is linked to the victim feeling "brainwashed", just an embryo of a subject there for the reader (you)

Fig. 3 (spiritualism, sexuality and life)
Spiritualism - seems to subconsciously spill out in relation to transgender/transsexual issues. Many I have spoken to feel that their 'soul' is of a woman. Now anyone can ask "what does that mean?" - I asked that too. We don't even know if souls are a thing. Scientists agree that human consciousness is all just an illusion caused by the combination of the neuron paths that control the functions of our bodies.
Subconscious - 'naturally acquired' meaning of depth found in spiritualism can also be born from childhood traumas. A child that is lost finding some meaning in a 'soul' in which they wish to reflect their identity, possibly including the exterior appearance.
Sexuality - Many over the age of 30 were gay men. Or straight women. A majority of under 30s seemed to have been straight men, or lesbian women. - This is from my tiny sample size of research (around 42 individual MTF people). Take from that what you'd like.
Life - So many variables. Finding a sexual partner post-op seemed to be an issue. One story from a lady recounts that she met a very handsome and wealthy man that loved her with a penis. She had the surgery and he decided he didn't like her anymore. Wealthy 'deviants' seemed to have been common figures in older peoples stories. Men intrigued by these 'exotic' brand of woman, almost.

Conclusion;
Klinefelters, low intelligence, autism paired with some kind of sexual trauma likely caused dysphoria for some people. Again there's still a 1% thing i cannot explain nor find an answer for. Maybe it is spiritual? Guess we will never know.
Redefinition of transsexualism/transgenderism was more damaging than helpful, as AGPs could slide in and use it for their gain.
