Like, at minimum, someone that’s dosing themselves and is on HRT should have their levels monitored at least every few weeks by a doctor. Your risk of cancer, heart disease, and stroke can increase wildly across both ends of the troon spectrum if your levels are fucked. Not only that, but they play a huge role in mood regulation. It’s like how sometimes cis women start hormone based BC and have wild mood swings for the first month because they’re suddenly inundating their body with a fat hit of estrogen and progesterone every day. Fuck, even depoprovera can spur suicidal ideation in otherwise hormonally normal cis women, even ones with no predisposition to depression. The fact that so many troons treat it with the same care as a one-a-day vitamin while also fighting for the right for minors to have access to puberty blockers is fucking BONKERS!

AND ANOTHER THING.... re: hormone blockers, in cis girls, if they’re started even slightly after the onset of puberty, they don’t fucking do anything. Age of onset for female puberty is generally between 9-12, sometimes even earlier if a child goes through precocious puberty. So, what, if your kid doesn’t know she’s trans by seventh grade she either has to ~*let the horrors of female puberty ruin her body*~ or with androgel?? (which, btw, carries a much higher risk of heart attack than injectable T)
I don’t see anything wrong with kids having access to counselors and psychotherapists that are competent in treating gender dysphoria, or even necessarily allowing them space to be GNC or to slowly socially transition with a wardrobe, hair cuts, and nicknames. All of that is reversible. But to give someone who can’t even sign off on their own medical paperwork the power to decide their fertility and physical well-being just seems like it’s a step too far.

off label prescribing is allowed in the US because when doctors are exercising discretion wisely, it can make a huge impact: for example, one of the early AIDS drugs, AZT, was an off label prescription of a cancer drug from the sixties. but it also means that troons won’t have recourse against the drug companies like middle aged men did against AbbVie: it’s not an approved use of their product and they’re not marketing it as such.

and it’s not like every drug troons take is as obviously connected to a previous inability to sell it like how the manufacturers of lupron got sued over selling it for precocious puberty. spironolactone, which MTFs use to suppress testosterone, is meant to treat high blood pressure. It’s available as a generic and costs less than a dollar a day in the US, so no drug company is getting rich off of it.

Even troons acknowledge this lack of research and its problems, as seen in Out magazine. And of course it also says many trans women want to maintain erectile function...so much for dysphoria, eh?

edit: people who live in a first world country and are getting drugs prescribed to them by someone with an MD and picking them up in an orange bottle from a licensed pharmacist are going to assume those drugs are basically safe. even if they know it’s off label they’re going to assume it’s like getting abilify (an antipsychotic) for anxiety, which is relatively common. but there’s not a physical difference in the bodies of different types of crazy people.

in my opinion the actual responsibility (and thus the problem) lies with the doctors and hospitals, because that's where all this off-label prescription fuckery gets approved and where the frankenstein surgeries happen, and also the insurance companies who greenlight the funding for it all.

This is going to be an interesting point of contention when this all comes to a head in several years. Doctors know, hospitals know, but you're living in a world where the troon agenda is 'reigning king' and especially so with this rise of the informed consent model of troonism when it comes to off-label hormone use and these surgeries. This isn't someone being treated for a real disease at this point where they're trusting their doctor and it goes horribly wrong- a lot of troons educate themselves in hug boxes and medical professionals have been bullied into silence over the complications that can happen.

As more people transition the scope of what will happen from these drugs and surgeries has the potential to manifest even greater numbers of future victims. The problem is when it all goes to hell, where is the blame going to go? because they certainly aren't taking any of it themselves. Then the medical establishment can tout their papers agreeing to them 'taking their chances' and agreeing to everything else as well, they pushed for it cried about it and NEEDED it. That sickness and body rot they're dealing with now, not the medical establishments problem.

At some point they're going to have to admit they're crazy and shouldn't have been indulged and boy will it be hilarious.
