I sometimes wonder how many of today's "on the spectrum" kids are really just abuse and/or neglect victims whose social skills are stunted by dysfunctional family dynamics.

It's a great way for the parents to get away with abuse, because they can always pull the "you don't know what it's like to have an autistic child, this is what works for our family and who are you to judge" card. Obvious warning signs of abuse, like sexually provocative behavior, bathroom issues, social withdrawal, will be seen as symptomatic of autism and not anything more sinister.

There is a document called the Coventry grid that gives a rundown of the similarities between symptoms of autism and symptoms of Attachment Disorder, they're similar enough to each other that a kid with an AD can easily be misdiagnosed. You can eventually grow out of an AD, although if it's the most severe form (Reactive Attachment Disorder) that kid is usually on the fast track to jail or other secure facility.

If I was to guess, I'd say this kid has a severe AD. Kids with even mild autism don't usually take well to uncomfortable clothes, slathering weird textures on their skin, bright lights, loud music and being around a large crowd. Kids with AD on the other hand can act out provocatively for attention in a way that's uncomfortably adult and lack boundaries around strangers.

There was a thing called Baby Burlesk made around 1932 with a three-year-old Shirley Temple, and it makes for as uncomfortable a viewing as you might expect. I was recently reading up on Complex Post Traumatic Stress Disorder. In children who have been abused over a long period of time, the only difference between CPTSD and Austism can be the lack of "special interests". The two are so similar that they're often mistaken for each other.
