The reason why they're so busy is that their schedules are bloated with responding to emails and meetings that they often prioritize over patient consultations.

I work under a pediatrician who could do 40+ patients a day. But he'll actively choose to do a nothingburger 2 hour meeting over zoom, and use it as reason to be only available for patients 2 hours of the entire day he'd originally work be disposable for 7 hours of. That's maybe 4 or 6 patients instead of 30 on average. You said their assistants can help with paperwork - I'm the assistant, the doctors prefer to do paperwork instead while I have to prep for patients.

Private facilities hold higher standards, but public clinics are horrible. The doctors have total power over the scheduling. A lot of them choose to go grab a coffee mid day with buddies and that's why you see random hour long gaps in available visits. Popular private clinics will actively occupy their doctors in order to fill up their schedules and reduce the number of available visits (especially during busy hours: ex. when most adults are off work and have the time to go to a doctor), this forces patients to pay for higher priority services and creates scarcity, so that you don't get too comfortable having a health professional with you 24/7 - it's not a good deal for them.
