We do not know what brought down the last F-15. In the two most likely cases, mechanical failure and enemy fire, it is statically likely going to happen at some point with enough sorties. Since the United States has apparently deployed both the B-1B and the B-52 to drop JDAMs it is likely that air defense is substantially suppressed/destroyed.
Edit: Israel just blew the air defenses up. I think it is more likely to be a mechanical failure here

We do not know what happened with the three lost over Kuwait, with reporting that it was Patriot or a F/A-18. What we do know is that Kuwait is small, the F-15Es were likely returning from mission, and they were hit in the rear of the aircraft. Air to air and Patriot missiles are super sonic, so reaction time can literally be seconds even from a distance. The strike to the rear suggests air to air to me. My understanding of PAC-2 missiles is they go above and down on the target, specifically going for the cockpit. That could just be mil-sperg meme lore though.

There's also a high likelihood the pilots heard the missile launch detectors go off so close and just slammed "eject" rather than try some risky shit.
We know all 6 pilots safely ejected so without more information about a specific instrument failure I'm not sure what the issue would be.
