Gun violence is caused by a large underclass that commits a disproportionate amount of crime with access to illegal guns. Mexico has the narcos and Ecuador and Brazil have a huge gang problem. America has a gang violence problem too but, it's smaller. Gun control doesn't mean shit.
Most mass shootings and shootings in general in the United States are gangbangers shooting each other. Go look at the "Gun Violence Archive"'s list of "mass shootings" (IIRC they have the vaguest definition of all the aggregates) and if you go through each of them its something like 95% of all shootings are blacks and hispanics doing hood shit.

I'm not familiar with the type of armor he was wearing, but I know some guys who wouldn't install the ballistic plate because it's heavy. Without the plate over your heart, the vest is just lightweight kevlar.

Depends on the Ammo. 7.62 FMJ/TMJ or other Armor piercers can still ventilate level 3. Level 4 can even be overcome with the right combo of charge, bullet weight, and jacketing.
ETA: Also, the bullet might not have pierced the armor, but blunt force trauma can still cause an embolism/ other bleeding from the orifices of the body.
