Maybe he’s conflating “vaginal wall” and the urethra? I should look at some diagram or paper again to see how a separate urethra and neovagina...

Perhaps the butcher has turned his urethra inside out to line his stinkditch, and the mass shown on the pictures is the prolapsed urethral wall?
