My money is on him being too fat during puberty and his body wasn't able to synthesize proper sex hormones so he came out more feminine than masculine. He's got little arms and legs like a woman, he's short of height, his dick is small, he has a severe excess of breast tissue and he doesn't appear to have much in the way of bodyhair. I'm sure if he got hormone replacement therapy he might be able to gain some kind of muscle mass but a lot of it (bone structure, tendency to carry weight on the hips like a woman, emotional pettiness,etc) isn't reversible.
