TL;DR - woefully misinformed and apparently sexist doctor completely unwilling to change anything in my treatment and making thinly veiled threats to cut me off if levels weren't at expected numbers.
Up front: I am not giving anyone medical advice, nor asking for any. Just relating my experience with my doctor.
Last time I saw my doctor's office, I mentioned that I wanted to raise my levels only a little over the WPATH method and follow something closer to another emerging method. To avoid breaking any rules here, I'm not mentioning specific amounts or where to find this research. The nurse I saw was convinced it was worth sharing with the doctor, so I got an appointment with him.
The man that finally walked into the office while I sat anxiously cradling my 3 subject notepad filled with facts, lab results, etc. was a very mature elderly gentleman with thick glasses. Right off the bat I felt he stared at me as one might a young, ignorant child that needs a good scolding. I stood up and shook his hand and thanked him for meeting with me. When we sat down, he started to talk through my file and I patiently listened. While waiting for a chance to talk, he quoted the medications and dosages I was on incorrectly, so in a moment of silence I tried to gently inform him what they actually were.
This is when the mansplaining and constant interruptions began. I barely got a word in. He was open about not being willing to try new medications. He used "all cis women" as proof that, if levels even lower than WPATH works fine for them, it's all the proof anyone needs that it works for all transwomen. He said raising levels would be risky, which I agreed with (although he kept making astronomical strawman arguments), but when asked to explain the risks or to go deeper, he quickly said "Well, we don't know, and wont for 40 years," before changing the subject. He went on about how women go on the internet and get all of this information and freak out and how it's all basically nonsense from people trying to make money with slick marketing, and that it's too difficult for a non-medical person like me. I didn't know where to begin after that statement.
When talking about cis women's menstrual cycles and the hormones associated he mentioned the peak was at a very low number - a very incorrect number. I had with me on my phone something published that showed higher numbers in cis women, and he dismissed it as too old. I mentioned it was from 2019 and he changed the subject again. He even went so far as to bring out a big fat medical manual from the 90's, and turned to a chart that directly contradicted him, and then with much hand waving dismissed his own chart as irrelevant.
I did my best to nod, give a friendly smile, be charitable in all my responses, never argue, only ask questions and emphasize that I wanted to work with him, learn from him and mitigate risks.
He told me flat out I needed to see another doctor if I wanted to change anything. He even said some ridiculous things about anti-androgens - like that they have zero side effects. To be fair I think he meant only the AA's they prescribe. Leaving that office, I felt condescended and scorned and even ridiculed. The whole thing made me a little sick to my stomach and made me feel like less of a person. I'd say more about what he specifically said, but I'm not sure how to do that without mentioning specifics about medications and such and my head is still reeling. Before I left, in an effort to make sure I wasn't dropped as a patient, I asked if we could keep things the way they are for now, please, and to continue seeing them, which they said was fine but added a thinly veiled threat if my levels got higher than they were comfortable with, I would have to find a new doctor.
I don't want to scare anyone and it may be that he is doing what he truly thinks is best for his transgender patients, but I'll be looking for another doctor.

Sounds like OP is going to take his three-subject notebook of true and honest facts to go doctor shopping.
There are asshole doctors, especially of the variety that don't listen and respond to patients' concerns. But OP was basically asking his already skeptical doctor to break out of standard transgender treatment and prescribe more of that sweet, sweet estrogen. I don't blame the doctor for not wanting to gamble with that. When a patient who is already healthy and is only taking estrogen to transition demands more estrogen, shit can hit the fan. How could a doctor be expected to take that risk for the sake of genderfeels?

From a fetish standpoint, there is probably a preference for different forms of tranformation. Some troons seem interested in hormones and surgery but some seem to really like the idea of pills magically turning them into a woman. Did this happen in anime or something? Idk.

From an aesthetic standpoint, the problem is that troons with enough body fat to mask the edges and rigidity of implants, being male, carry their fat in a male way, which generally means more of an apple shape. That means they have no good way to create a waist and hips. I mean there was that one fat troon who looked like he’d strapped a dinner plate to each hip, but that is not a good way to do it,

Oestrogen, despite its many many side effects, is still tolerated better by the human body than drugs used to treat (non gender related) dysphoria, which are commonly drawn from antipsychotics and anti seizure medications.

Also despite what the sexist shitlords will tell you, you are perfectly capable of operating heavy machinery, driving motor vehicles and making legally binding decisions while on oestrogen.

When there is not enough breast tissue to accomodate an implant, the popular surgical procedure is to cut through the pectoral muscles and insert the implant underneath. This has a high rate of complications and is generally not recommended, as in the surgeon will actively talk your implant size down.

How good an implant looks depends on the fucking restraint on the part of the patient not to choose fucking melons.
