Saline/silicone basically fills in the scrotal "void" around the testicles, so even though they appear very large what you end up with is not unlike putting an M&M inside of an inflated balloon. The fact that it just fills in said void is why more often than not it turns the organ into one giant sphere instead of something with two distinct balls inside of it. They still "work" because the injected substances are inert within the human body, but the substances can (and will) migrate within the body which is why in the nude pics of Dylan he's wearing a sort of ring around the top of his nutsack. I've actually wondered this before, about why there are so many gay men with extreme/gross/fucked up fetishes.

Degeneracy breeds degeneracy, so I imagine it has something to do with when homosexuality was heavily demonized as a disgusting paraphilia. The men who wanted to have missionary sex with their husband while the lights are on were grouped with people into BDSM and similar things. The only way for these men to get any kind of sex would be to associate with these sexually depraved people and had gotten roped into whatever scat prolapse sex they were having. And if society saw gay vanilla sex as being basically the same as jerking off with vomit, why would you bother trying to maintain a normal sex life?
It's well known that sex was prominent in the early days of American LGBT activism because their identities were described as being an abnormal fetish. Since LGBT activism grew out of abnormal sex and gay men were the ones especially having the most depraved sex, it continues to be common with gay men.

This. It’s typical cult behavior of targeting people with low self esteem and then isolating them and making them dependent on the cult leader. Even if they want to leave, he’s financially in control, they have no friends to help and his past behavior has shown he is willing to sue them and attempt to ruin them if they leave.
It probably helps that they have the same bizarre fetish. Most people aren’t into the disfigured He-Manlet with hydrocele look.
 
After reading (parts of) the OP I come to the conclusion that I'd be ok with everyone involved dying.

Also I assume he's probably some overpaid Silicon Valley IT something-something and just has money. That's how he ropes young guys in. (It's also the only industry in the only part of the world I could think of where you could earn lots of money and being accepted for looking like that freakshow)

Maybe Trump can come to a mutual agreement with Russia that they'll nuke San Francisco.
