I think Nick is either drinking or is abusing prescription meds, (he has either a doctor over prescribing him or is still using his dealer, but now for prescription drugs)

The nitrous and the amount he is doing could be responsible for the rosacea and the nose. He could be drinking but all the stuff he is doing with nitrous and vaping says to me that he is almost constantly fighting the urge to drink. I also kind of think he isn't the type to be able to drink just a little. If he started drinking again, he would quickly spiral and couldn't really control it.

The human can get away with a few days of piecemeal 3 hours here and 4 hours there, but a single night of sleep deprivation does measurable harm to the immune system and other processes, definitely including cognition. Stack that up for weeks on end and you get real problems. Add a cocktail of chemicals and the dopamine mind of doomscrolling and you probably get what we see.
