It’s gross to think about but my guess would be maybe he’s able to once a month. He seems extremely medicated so I know it’s messing with him. I feel like Ethen's scribble tattoo is a litmus test on whether or not you should interact with someone. Any who reacts negatively to it shouldn't be interacted with. It's a precious memory a father has for his child but no "It's cringe and gay, look at this shitty knife and naked woman. Aren't I cool guys? My wife says so."
If you have a custom tattoo idea, you would typically shop around and find an artist skilled in that style or who's done similar work before. The 'original tattoos look bad' cope is only true if you have poor taste or are a bad judge of others' expertise, both of which apply to Ian. What a surprise that he's projecting again.
The Gumroad archive is very hit or miss, it's missing a lot of content. Everything before December 2023 is just missing from the archive. As well, they just to forget to upload some shows all together on the Gumroad, the most recent notable example is the show they reacted to the AVGN Movie.
