Based on my experience of knowing people in this situation they will sometimes get existing pieces reworked into new (hopefully less shit) designs but a lot of the time they get 'blackout' tattoos to cover up the old shitty stuff they no longer like. They look like this and are absolutely horrendous:

I can also say from personal experience that this would be agony to get done. Solid black in a tattoo, even in small areas, is a miserable experience because if you want it done properly it means going over the same area again and again to ensure full coverage, so the skin is red raw and physically throbbing by the time it's done.
