Along with all the other ways it fries your brain, heavy alcohol consumption can (sort of ironically) make you more anxious and paranoid long-term. It's one of the reasons alcoholism is so insidious, you drink to get rid of anxieties and end up making them worse in the end. Some of Pat's behaviour is definitely unhinged enough to be him abusing uppers, but it could also easily be the booze plus his withered soul finally departing his body once he paid Quasi, leaving him a broken shell of a pig man.
