I mean, it's obvious the painting's subject is meant to look masculine and uncanny. You may even read it as “men in women's clothing look disgusting and we should mock them.” Flemish artists were TERFs.

After quickly reading Wikipedia, the disease hypothesis was made in 1989 by doctors who don't understand art is rarely meant to be taken literal. This is obviously a work of satire and, while it may have been inspired by the canivalesque crossdressers (meaning: a man dressed as a woman because it's funny) or a particularly ugly old woman, I doubt the painting had a singular model. There seems to be various caricatures circulating during the Renaissance of a similar grotesque figure.

Crazy how 500 years ago this was mocked. Something so insane you would need an artist to portray it. Today it's heckin' valid.
