I strongly suspect so. As I’ve said elsewhere, he seems to be having real trouble making sense of things. Like, he’s contradicting himself even within the same sentence. He’s always been kind of deranged, but he seems far worse than usual. I bet he’s never really been so far as decided to use even go want to do look more like.

It's pretty well known in the medical community that long term abuse of drugs can speed up the onset of dementia. Combine that with his untreated schizophrenia (which also accelerates the onset of dementia) and Tom's brain is just a ticking time bomb at this point.

The COPD isn't helping. Long term oxygen deprivation, made worse by continuing to smoke. There's just no way to make up the oxygen deficit after a while, and the effect is cumulative.

I am not sure at what point you get complete congestive heart failure, but that's got to be close.
 
He has osteo perosis for sure . Look at his posture. I also think he may have diabetes mellitus and is not managing it. While the opioids theory makes a lot of sense, diabetes could explain it too. It’s difficult to know whether the browning at the end of his fingers could be diabetes or just general filth. The eye twitching could also be explained by unmanaged diabetes.

The bone recession in his face is pronounced. Take a look at pictures of him even 3 years ago and the difference is striking. This is no doubt due to very poor dental hygiene and smoking. Diabetes would be a major accelerant of this process too.
 
No matter how you put it, drugs are a poison. If it is affecting your cognitive function, it is affecting the rest of your body as well.

Sooner or later, the cumulative effects emerge as health problems. There is a reason drug addicts age quickly.
 
Definitely, I work with addicts regularly and they all have that signature face. Give it ten years max and Tom will be a corpse.
 
Tooter is in his late 60's. Constant contact with filth, a trash diet, drugs, and stress. I'm not sure if he's the cockroach that could survive nuclear fallout, anymore. His neck and lung problems are telling. If the body doesn't give out, his mind will first.

Tom has all the signs of antisocial, histrionic, and schizotypical personality disorders. Having both type a & b clusters enables dementia to take an easier stranglehold on the sick. At maximum lifespan, I give him 10 years; Becoming a husk at the end.

Drugs also have a serious deleterious effect on sleep and sleep quality. Even if Tom was only a demented stoner, marijuana addiction and chronic use causes major sleep issues. Sleep is vital to healing. Skin health and vitality is dramatically impacted by sleep quality over time.

Also eating out of the garbage slowly turns you in to a raccoon so he has that problem too.

I doubt it's just the early stages. He has to be pretty far in. When my grandfather was in his final years, his dementia made him say grandiose things like he was a center player for the Toronto Maple Leafs. And we all know what Tom likes to do, proclaim himself as a Grammy Winner, a licensed marine biologist, etc. It's even evident in his posts here where he contradicts himself twice, maybe three times IN THE SAME SENTENCE.

So my answer would be yes, just not in the way we'd seemingly all think.

EDIT: Please keep in mind I'm not an expert, let alone a doctor, Mari is probably more qualified to speak on this matter than I am for all I know, I'm just going by personal experience and also all the evidence there is here. But I highly doubt it takes either to know that the next few years probably aren't gonna be pretty for him. People here are saying 10 years, I'd say that's pretty optimistic and probably realistically more around 3 or 4.
